COVID-19 Screening Questions

We look forward to seeing you at your upcoming appointment. Here are a few things to remember as you prepare for your appointment. Please read the instructions carefully.

 

Standard COVID-19 screening questions:

 

Have you or any member of your household experienced any of the following symptoms in the

last 14 days? If you answer “yes” to any of the following, please contact us to reschedule your

appointment.

• Cough or sore throat

• Fever of 100.4 degrees or greater

• Decrease sense of taste or smell

• Experienced shortness of breath

• Muscle aches or pains

• GI symptoms (nausea, vomiting, diarrhea)

• Travel outside of the state or country

• Come into contact with a person who has tested positive for COVID-19

 

Important office protocols as we follow CDC guidelines:

 

• Call/text us from your vehicle when you arrive to check in (801-544-3323)

• Siblings without an appointment are NOT permitted in the office

• Please escort your child into the building once we notify you when we are ready

• A KPDO team member will screen you and your child for COVID-19 symptoms and take

your temperature.

• One parent/guardian will be permitted with the patient into the office IF they are

wearing a mask (please bring your own mask, we are unable to provide them)

• Public areas of the office (except restroom) will be closed and no one will be allowed to

sit or congregate in the reception area

 

Each patient entering the office will be required to do the following:

 

• Hand sanitize

• Rinse with hydrogen peroxide solution for 30 sec

• Be escorted directly to the chair for treatment

• Dismissed as soon as treatment is complete

• Upon completion of the treatment visit, the patient will be escorted to the front door and “handed-off” to their parent/guardian

• If payment is due, instruction for paying online at our website will be texted/emailed or payment information will be taken over the phone.

• Follow-up appointments will be made through text or phone call
